Why I Started Forever Rooted

I worked home health as a physical therapist for years, and saw the same thing over and over: people who wanted to stay in their own home, but couldn’t, because of health issues or because the home itself wasn’t set up for it. Nursing homes have stayed over capacity the whole time, so waiting for a spot to open up usually isn’t a real option.

Nursing homes and skilled nursing capacity in the U.S. isn’t keeping pace with demand. Forever Rooted exists to give older adults a real alternative: a clinical, evidence-based path to staying safe, strong, and independent in their own home for as long as possible.

My family and I put down roots in Montana for a lot of reasons, independence and self-reliance among them. I started Forever Rooted to use my skills and passion as a Doctor of Physical Therapy to help people stay in their own home, safely, longer. I’ll work through this with you step by step, to make sure you’re able to remain in your home on your terms.

Jason Makoutz, PT, DPT

Doctor of Physical Therapy, licensed in Montana. Background in home health and geriatric care, with clinical experience assessing fall risk, mobility, and functional independence in older adults. Assessments are built on the same evidence-based tools used across the healthcare system, including the CDC’s STEADI framework, the Short Physical Performance Battery, the Berg Balance Scale, and the Tinetti Performance-Oriented Mobility Assessment.

Outside of work, I’m a husband and father. Faith, family, and the mountains are what keep us grounded here in Montana. Most weekends you’ll find us skiing, fishing, hiking, or just outside. Helping people stay rooted in their own homes, on their own land, for as long as possible isn’t abstract to me. It’s the same thing I want for my own family, decades from now.
